Eligibility

Your child may come to the clinic:

► If you are uninsured and the family income does not exceed 200% of the Federal Poverty Guidelines. This will be assessed at the initial visit.
► If you have Straight Medicaid, Simply Health Medicaid, Sunshine Health Healthy Kids, Ambetter from Sunshine Health, Aetna (HMO), Cigna (HMO), United Healthcare (HMO), Molina Medicaid, TRICARE and are assigned to the clinic.
►If you have Cigna PPO, Aetna PPO, Blue Cross/Blue Shield PPO and United Healthcare PPO.
